About The PCN

The Wood Buffalo PCN was established by Fort McMurray’s family physicians in 2006 to increase access to primary care for local residents.

Every family physician in Fort McMurray is a member of the Wood Buffalo PCN. The PCN maintains a Central Clinic team comprised of allied health professionals and administrative staff that help to support quality improvement efforts in local family practice clinics and deliver comprehensive chronic disease management and preventative care programs.

Position Summary

The Wood Buffalo Primary Care Network is currently seeking a temporary 1.0 FTE Practice Facilitator (PF). The Practice Facilitator provides comprehensive support services to physicians, clinic managers, panel managers, and other clinic staff across the Primary Care Network. The Practice Facilitator will be an integral member of the care team and be responsible to identify and track patients attached to the practice population, assist with the proactive care of these patients, and manage related data and reporting relevant to the practice population.

What You Will Do
  • Work collaboratively with physicians and their teams to identify, plan, and implement improvement solutions
  • Participate in the care of individual patients, practice populations, and associated communities while implementing the Patient Medical Home model of care
  • Assist and report on clinic panel, including clear articulation of process
  • Be a key team member in panel identification/management; screening and prevention programs, clinical program improvements, EMR standardization and use, and other initiatives as the program evolves
  • Provide facilitation, consultation, education, and mentoring/coaching to clinical staff as teams work to identify and implement quality improvement processes
What You Need To Succeed
  • Experience in family medicine / primary care clinic setting
  • Experience with Electronic Medical Records
  • Excellent written and verbal skills
  • Outstanding computer knowledge; including Microsoft office suite
  • Work well with others
  • Excellent problem solving / critical thinking skills
  • High school diploma or equivalent with a related post-secondary certificate or diploma (Medical Office Assistant, Health Information Office Assistant, or similar) is required
  • Minimum of two (2) years of medical office related experience is required. Please note that candidates with less than two (2) years of experience will not be considered
  • Proof of a clean criminal record is required
  • Completion of reference checks
  • Must have own vehicle and valid driver’s license, this is a multiple site position
