Heart & Stroke Health Check
To access this program, you need a referral from a family physician. If you do not have a local family physician, visit Alberta Find A Doctor to search for physicians accepting new patients.
The Health Check Class is intended for patients who have been referred to the Wood Buffalo PCN for primary prevention (risk reduction and education) or secondary prevention (post-event). This means that each class will be a mix of patients who have had heart events or strokes, and patients who are at risk for such health problems.
We group these two subjects into one class, Strokes and Cardiac Events (e.g. Heart Attacks), because both are complications caused by vascular disease, damage to the veins and arteries.
Participants will have the opportunity to learn about Heart Disease and Stroke, associated risk factors, as well as risk reduction strategies. Other WB PCN services that can help to reduce risk and to recover from one of these health events will also be discussed in detail.
The Health Check class is taught by a wide variety of healthcare professionals who all have expertise in different areas of heart and stroke care. Your class may be taught by a combination of pharmacists, nurses, dietitians, and an exercise specialist.
To participate in the Health Check Class, ask your family physician if you are eligible for a referral.
