Healthy You

Your Health. Your Team. Your Choice.

Because every person has their own unique challenges and goals, we tailor our services to each individual. We encourage our patients to play an active role in designing care plans that work for them. You set the pace that is comfortable for you and choose which health professionals you want to work with. Our Healthy You program is designed so that you can work one-on-one with a member of weight management team (nurse, dietitian, exercise specialist, pharmacist, mental health therapist) for ongoing support and/or attend our group classes.

The Healthy You 12-week lifestyle group is originally developed by the Hamilton Family Health Team and helps you to acquire knowledge on ways to better your health through more mindful food choices, physical activity, and feeling good about yourself. Our Healthy You team will provide education regarding the following topics:

  • nutrition for healthy eating (important nutrients, vitamins and minerals)
  • reading food labels
  • increasing activity and incorporating resistance training
  • meal planning
  • dining out and grocery shopping tips
  • mindful eating
  • goal setting to help become your healthiest you

Healthy You follows the Health At Every Size ® (HASE®) approach to wellness.  If you are interested in accessing this program, please see your Family Physician to obtain a referral.
